Abstract: OBJECTIVES: To determine the frequency and characteristics of repeated unprotected anal intercourse with casual partners UAI-C). METHOD: Gay men were interviewed three times (at yearly intervals) for the ongoing cohort study, Sydney Men and Sexual Health (SMASH). RESULTS: In all, 577 men have completed three SMASH interviews. Of these men 451 (78%) engaged in anal intercourse with casual partners; most of it condom-protected. For the three six-month periods prior to each interview, 166 men reported having any UAI-C during only one of the six-month periods; 59 reported any UAI-C during two periods; and only 18 men (3% of the sample) reported any UAI-C repeatedly. Demographic characteristics of these 18 men varied widely. At each interview, at least half of them reported having casual partners only (i.e., no regular partners). Their HIV status was 10 positive, 6 negative and 2 unsure at outset, and 12 positive, 5 negative and 1 unsure at subsequent interviews. The non-positive respondents had non-positive casual partners only, and the positive respondents had casual partners of positive (mainly) or unknown status. CONCLUSION: Few gay men engage repeatedly in UAI-C. Those who do tend to be positive men and much of their UAI-C is with other positive men (posing no HIV transmission risk to an uninfected person).
